Compensation Local Pay:
- $3200/week - local weekly gross pay
- $3500/week - travel weekly gross pay
Responsibilities:
- Provide comprehensive discharge planning and utilization review services.
- Collaborate with interdisciplinary healthcare teams to facilitate patient care and discharge planning.
- Monitor patient progress toward discharge goals.
- Coordinate post-acute care services and community-based resources.
- Make timely referrals and ensure continuity of care.
- Work closely with patients, families, and healthcare providers to achieve optimal outcomes.
Requirements:
- Active RN License.
- Recent Hospital Case Management experience required.
- Strong discharge planning and utilization review experience.
- Experience working within interdisciplinary healthcare teams.
- Ability to coordinate services with post-acute care providers and community organizations.
- Immediate availability preferred.
