Medical Case Manager 7/6 START
Greenport, NY

Duration: 13 Weeks with the possibility of extension.
Schedule: Monday – Friday, 8:00 AM – 4:00 PM

Compensation Local Pay:

$3200/week - local weekly gross pay
$3500/week - travel weekly gross pay

Responsibilities:

Provide comprehensive discharge planning and utilization review services.
Collaborate with interdisciplinary healthcare teams to facilitate patient care and discharge planning.
Monitor patient progress toward discharge goals.
Coordinate post-acute care services and community-based resources.
Make timely referrals and ensure continuity of care.
Work closely with patients, families, and healthcare providers to achieve optimal outcomes.

Requirements:

Active RN License.
Recent Hospital Case Management experience required.
Strong discharge planning and utilization review experience.
Experience working within interdisciplinary healthcare teams.
Ability to coordinate services with post-acute care providers and community organizations.
Immediate availability preferred.
